In both, ethanol-mediated neurodegeneration was linked to insulin/IGF-1 resistance, persistent oxidative stress, and impaired acetylcholine homeostasis.64 These pathophysiological processes promote progressive cell loss, degeneration, and impairments in organ/tissue function. The role of insulin/IGF resistance vs. oxidative stress as mediators of ALN is under investigation, but given its role in alcohol-mediated disease in liver and the central nervous system it makes sense that it would also likely be a factor in ALN. By identifying and understanding the primary target and underlying mechanism of alcohol neurotoxicity, more effective—and possibly curative—treatments other than alcohol cessation, improved nutrition, and thiamine treatment may be developed.
Gastrointestinal symptoms include delayed stomach emptying and intestinal transit, dyspepsia, and faster emptying of the gallbladder [165]. Besides, approximately 55% of men with AAN develop erectile dysfunctions [167]. Cardiac arrhythmias in patients with AAN might increase the probability of sudden cardiac death, which is probably due to toxic effects of alcohol on a cardiac muscle that is also observed in alcoholic cardiomyopathy [168, 169].
Alcoholic polyneuropathy
Alcohol-related peripheral neuropathy appears to be characterised by severe loss of myelinated fibres; and although profound small fibre loss can also be present, this appears to occur more variably [3, 51, 53, 59, 85]. The data indicates that there is both small and large fibre loss in alcohol-related neuropathy, but that small fibre loss is generally predominant [3, 51, 53, 56, 59, 63, 86]. An analysis of lab data may correlate with the patient’s neuropathic syndrome and systemic symptoms. alcohol neuropathy stages Elevated levels of the liver enzymes gamma-glutamyl transpeptidase, aspartate aminotransferase, alanine aminotransferase, and alkaline phosphatase may indicate long-standing heavy alcohol consumption and its effects on hepatic function. Decreasing albumin, increasing bilirubin, and prolonged clotting factors may indicate hepatic decompensation. Red blood cells (RBCs) tend to be larger than normal (macrocytosis) and reduced in number from a deficiency in vitamin B9 or B12 or GI bleeding.

Treatment for Alcoholic Neuropathy
They may have peroneal nerve damage, causing foot drop with difficulty raising the feet sufficiently to walk safely without stumbling. Patients may have difficulty buttoning a shirt or blouse and with handwriting and holding objects. Patients may also have a deficiency in vitamin B12 (cobalamin), affecting the axon and causing muscle weakness, sensory disturbances, and anemia. Vitamin B9 (folic acid) levels tend to be decreased, reducing the density of small and large nerve fibers.
Additionally, the tactile sensitivity reflex was observed in thinner monofilaments in the AL group. Despite being considered as axonal-predominant neuropathy, Koike et al. (2001), important abnormalities in the myelin sheath after chronic alcohol consumption are observed, such as irregularities and demyelination, with enlargement of the nodes of Ranvier (Koike et al., 2001). Alcohol-induced PN can be managed with antiseizure medication, which has an anticholinergic effect on the central and peripheral nervous systems and blocks the uptake of serotonin and norepinephrine to decrease pain perception. Gabapentin, pregabalin, and carbamazepine are commonly used to alleviate burning and stabbing dysesthesias. Tricyclic antidepressants, including amitriptyline, desipramine, and nortriptyline, work similarly to the antiseizure medications.
